Regardless of preexisting lung illness, patients suffering from liver cirrhosis, especially decompensated can develop distinct pulmonary complications. Liver cirrhosis should be assessed for hepatopulmonary syndrome (HPS), portopulmonary hypertension (PoPH), hepatic hydrothorax (HH) and spontaneous bacterial empyema (SBEM) which are the most clinically significant consequences, in particular when dyspnea develops conjunction with cirrhosis. These entities differ terms pathophysiology, clinical characteristics, diagnosis suitable treatment options. This emphasize need specific diagnostic algorithm presenting or other symptoms. complications might rare portal hypertension, but these carry morbidity mortality risks and, therefore, strong suspicion is required to make an early accurate diagnosis. There several medical therapies available each condition multiple studies treatments procedures does not have benefit short lived benefit. The only that changes prognosis effectively long term transplantation. However, transplantation also needs careful considerations as on some cases it increase risk mortality.
